Individual and Family Supports at The Arc Service Coordination is a person-centered planning approach, designed to help individuals get necessary services and support, based on their individual needs and desires. Family Support Services include in-home respite to provide a break for caregivers, out of home recreation opportunities for socialization, and family reimbursement to help with expenses specifically related to the care of a family member with a disability living at home. Home Care provides hands-on help for bathing, dressing, meal preparation feeding, household and other types of chores to relieve the primary caregiver. Guardianship Services ensure that people over 18 years of age with developmental disabilities have someone to act on their behalf and in their best interest, without removing the person's other civil or legal rights. Transportation is an integral part of bridging support available to people with developmental disabilities. Transitional Planning, particularly from School to Work or from School to other life experiences. is another phase of Service Coordination and person centered planning available through The Arc. Day Habilitation assists adults with disabilities to acquire, retain or improve self-help, socialization, and adaptive skills and to help develop motor skills.
